Diabetes Self-Management Education and Support (DSMES) and the National Diabetes Prevention Program (National DPP) are proven programs that help people make lasting changes. Yet many patients never get referred, or don’t know these services exist—especially in rural and underserved communities.

This 1-hour live webinar gives Ohio healthcare professionals practical ways to identify eligible patients, refer them, and partner with DSMES and DPP providers to improve outcomes.

  • Apply interprofessional, team-based strategies to integrate DSMES and DPP into clinical and community workflows across Ohio. 
  • Evaluate the impact of DSMES and DPP on patient outcomes, including prevention, glycemic management, cardiometabolic risk reduction, patient activation, and health equity.
