Learning Objectives

  1. Explain how hormonal changes in the menopause transition contribute to mood, sleep, and cognitive symptoms, and use a severity-based framework, from subclinical depressive symptoms to a major depressive episode, to guide treatment selection.
  2. Identify and differentiate depression, anxiety, and bipolar disorder in perimenopausal and menopausal patients, and select and sequence validated screening tools (PHQ-2/GAD-2 or PHQ-4, PHQ-9/GAD-7, MRS, and MDQ), including the MDQ before starting an antidepressant.
  3. Evaluate and individualize antidepressant, menopause hormone therapy, nonhormonal, and behavioral treatment options, and combine them into a patient-centered care plan.
  4. Carry out a same-visit safety protocol for suicidal ideation, including risk assessment, escalation, and documentation.
  5. Apply a team-based integrated behavioral health framework, including collaborative care, to coordinate care with other professions and behavioral health partners and to match the level of care to the patient's needs and risk.
