Overview
This learning collaborative is designed as a quarterly, interprofessional CE-accredited series that fosters innovation, shared learning, and peer-to-peer support. By engaging community and chain pharmacies, the program helps build sustainable infrastructure for DSMES/DPP delivery, improves access to care, and strengthens outcomes for priority populations across Ohio.
Target Audience
- Primary: Pharmacists and pharmacy technicians (with or seeking DSMES/DPP accreditation/recognition)
- Secondary: Clinicians and care team members who refer to DSMES/DPP programs
Learning Objectives
By participating in this series, learners will be able to:
- Strengthen interprofessional team collaboration to expand DSMES and DPP delivery in community pharmacies.
- Apply strategies to improve access to care and patient engagement, especially in high-burden and underserved populations.
- Interpret and utilize data to understand and impact patient outcomes in diabetes prevention and management.
- Integrate sustainable practices (billing, accreditation, retention strategies) to ensure continuity of DSMES/DPP programs.

JOINT ACCREDITATION STATEMENT
In support of improving patient care, Innovation Horizons is jointly accredited by the Accreditation Council for Continuing Medical Education (ACCME), the Accreditation Council for Pharmacy Education (ACPE), and the American Nurses Credentialing Center (ANCC), to provide continuing education for the healthcare team.
IPCE – ACCREDITATION STATEMENT
This activity was planned by and for the healthcare team, and learners will receive 4.0 Interprofessional Continuing Education (IPCE) credit for learning and change.
